Valley's Best Hospice is actively seeking a full-time RN Case Manager to join our rapidly growing clinical team providing best-in-class hospice service to our patients and their families. Be the critical part of our team and make a difference by helping those with life-limiting illness to live their best life.

As an RN Case Manager, you will lead the Interdisciplinary Team of highly skilled team of Nurses, MSWs, CHHAs, Spiritual Counselors, and Volunteers. Your low case load ensures you are able to provide compassionate care, active symptom management and emotional/spiritual support. You will be supported by experienced clinical hospice leaders while working for a prominent hospice provider organization, with opportunity for advancement.

Responsibilities Include:

• Admit patient and coordinate all aspects of their care
• Provide Skilled Nursing Care in a home or facility setting
• Teach and ensure appropriate use of medications, DME, and supplies
• Educate and support patients, families, and caregivers in the Hospice Philosophy
• Participate in Interdisciplinary Team Meetings and updatie the Plan of Care
• Provide our patients and families the best care and support possible

Job Requirements:

• Valid California RN License
• Current BLS
• Preferred 1 years of experience in either Home Health or Hospice within the last 3 years
• Valid driver's license and automobile insurance. This is a field position. 
• Ability to travel throughout the Orange County and Riverside County required
